GSGA Adaptive Golf: COVID-19 Release, Waiver and Covenant Not to Sue

As a condition to participate in Georgia State Golf Association (GSGA) Adaptive Golf Program events through December 31, 2020, a COVID-19 Participant Release, Waiver and Covenant Not to Sue must be signed and received by the GSGA prior to the individual's participation in any GSGA competition or related activity.

Note: A parent or legal guardian is required to sign the COVID-19 Participant Release, Waiver and Covenant Not to Sue for any individual under the age of 18 years old.

If this Release, Waiver and Covenant Not to Sue is not signed and returned in time, the participant will not be permitted to participate in any GSGA activities until the document is signed and received by the GSGA.

This Release, Waiver and Covenant Not to Sue may be emailed to Colton Dean at cdean@gsga.org, faxed to (770) 955-1156 or mailed or delivered overnight mail to GSGA (Attn: Colton Dean) at 121 Village Parkway NE, Building 3, Marietta, GA 30067.

The Georgia State Golf Association (GSGA) is the governing body of golf in Georgia and serves as an Allied Golf Association to the United States Golf Association (USGA).
