As a condition to participate as a player or a caddie in Georgia State Golf Association (GSGA) competitions through December 31, 2021, a Participant Release, Waiver and Covenant Not to Sue must be signed and received by the GSGA prior to the individual's participation as a player or caddie in any GSGA competition or related activity. Junior participants will complete acknowledgement of the waiver through the online tournament registration process and do not need to submit a signed waiver document. Adult partners in team events and caddies must still provide the GSGA with a signed waiver document prior to their participant’s starting time on the first day of competition.
Note: A parent or legal guardian is required to sign the Participant Release, Waiver and Covenant Not to Sue for any player or caddie under the age of 18 years old.
If this Release, Waiver and Covenant Not to Sue is not signed and returned in time, the participant will not be permitted to participate in any GSGA activities until the document is signed and received by the GSGA.
This Release, Waiver and Covenant Not to Sue may be signed and emailed to Jeremy Freyman at jfreyman@gsga.org, faxed to (770) 850-9009 or mailed or delivered overnight mail to GSGA (Attn: Jeremy Freyman) at 2205 Northside Drive, Suite 200, Atlanta, GA 30305.
